PAGCL (post-arthroscopic glenohumeral chondrolysis) is a cartilage-destroying condition that produces pain, reduced mobility, and other problems. It may occur in the shoulder, knee or ankle. A significant number of patients have developed PAGCL after being treated with a shoulder pain pump following arthroscopic shoulder surgery.
As reported in 2006 by the American Academy of Orthopedic Surgeons of the more than 150 individuals examined who underwent arthroscopic shoulder surgery between January 1, 2003 and December 31, 2005, 12 unexpectedly developed post-arthroscopic glenohumeral chondrolysis. The only common factor among these 12 patients was their post-surgical treatment with a shoulder pain pump.
The term “glenohumeral” refers to the shoulder joint, a ball-and-socket joint that is prone to several types of problems that may require surgery. The term “chondrolysis” means cartilage deterioration, a process that cannot be reversed. “Post-arthroscopic” refers to the time when the condition develops: after an arthroscopy procedure.
A 2007 study published in the American Journal of Sports Medicine also investigated the cause of PAGCL, which could be considered a fairly new disease. The arthroscopic shoulder surgeries that are sometimes followed by the use of a shoulder pain pump are themselves relatively new; the use of arthroscopy is an alternative to “open surgery,” using very small incisions and a laparoscope (a thin, long tube with a tiny camera).
Shoulder pain pumps are also rather new. They’re designed to deliver pain-killing medication such as lidocaine through a catheter directly into the shoulder, until the pain is manageable.
