The treatment of PAGCL (post-arthroscopic glenohumeral chondrolysis) involves efforts to end the pain and reduced joint mobility that this condition brings. PAGCL is a painful, debilitating deterioration of cartilage in the shoulder (glenohumeral) joint. Some PAGCL patients never regain the complete use of their shoulder joints, and the pain they’re experiencing may never go away.
The causes of PAGCL are not yet completely understood. PAGCL is in the news nowadays because many patients are being unexpectedly diagnosed with this condition following arthroscopic shoulder surgery and the use of a “shoulder pain pump” that administers medication.
The treatment of PAGCL is of particular concern to those who have a severe case of this condition, which can be extremely painful and long-lasting — even permanent. A first-line treatment for PAGCL pain is an analgesic (an agent that eliminates or reduces body pain). Some PAGCL patients may be able to control their pain and other symptoms with analgesics and/or other drugs (e.g., anti-inflammatory medicines), but they may have to take these medications indefinitely, since PAGCL does not go away on its own.
PAGCL has stricken not only elderly patients; even people in their 20s have developed PAGCL following the use of a shoulder pain pump. In such cases, the afflicted may need to take medications for years as a treatment for PAGCL’s effects, and the relief afforded by medications may not be complete. In addition, heavy pain medications and other drugs have their own drawbacks, side effects and risks.
The cartilage in the shoulder joint that is destroyed by PAGCL will not regrow. If pain medication is not sufficient to help a PAGCL sufferer, a replacement of the entire shoulder joint — that is, a shoulder arthroplasty — may be the next appropriate attempt at treatment for PAGCL.
In a shoulder arthroplasty, the shoulder joint is replaced with parts made of plastic and/or metal. Arthroplasties are full surgeries, performed under general anesthesia and requiring a significant recovery period. Physical therapy to regain use of the shoulder and arm after an arthroplasty will be necessary.
