Intra-articular pain pumps (for example, On-Q pumps) have been linked to a debilitating condition known as PAGCL, or post-arthroscopic glenohumeral chondrolysis. The intra-articular pain pumps are prescribed after arthroscopic shoulder surgery to temporarily deliver pain medication, but PAGCL is itself a very painful condition that has been diagnosed in an alarming number of patients treated with an intra-articular pain pump.

In PAGCL, the cartilage in the shoulder joint begins to deteriorate (chondrolysis). Cartilage is a smooth, firm, whitish tissue that covers the ends of the bones at the joint, acting like a shock absorber. Shoulder cartilage damage causes a great deal of pain, and this pain may not be controllable with medication.
In fact, the cartilage destruction caused by PAGCL is not reversible. The only plausible treatment for a severe case of PAGCL is additional shoulder surgery.
