Reglan syrup (that is, metoclopramide oral solution) comes in pints, as an orange, sugar-free and preservative-free liquid. The syrup has also been referred to as an "oral concentrate" or "concentration." A single dose of Reglan syrup (one teaspoonful) has five milligrams of the active ingredient, metoclopramide, an anti-emetic and prokinetic agent.
According to the makers of Reglan syrup (which have included A.H. Robins and Schwarz Pharma), this prescription-only medication is indicated for adults with gastroesophageal reflux who have failed to respond to other therapy. The symptoms of gastroesophageal reflux are:
The use of Reglan syrup for gastroesophageal reflux or any other purpose — such as diabetic gastric paresis or nausea/vomiting — is approved by the U.S. Food and Drug Administration (FDA) for certain adults, but Reglan in infants or pets is not approved.. Nevertheless, Reglan syrup has been administered to both infants and pets as a treatment for nausea or reflux. An overdose of Reglan syrup is a definite possibility in either an infant or a canine/feline patient, because Reglan does not come in prescribed doses for these patients.
In addition, Reglan syrup and other forms of Reglan/metoclopramide are indicated only for short-term therapy — that is, for four to 12 weeks. The risk of serious adverse Reglan side effects occurring is much greater when Reglan syrup is prescribed for longer terms. Unfortunately, some patients have been prescribed Reglan syrup for periods as long as one year or more.
In early 2009, the FDA issued a Reglan warning to physicians and patients, warning that Reglan products (including syrup) are linked to a very debilitating movement disorder, tardive dyskinesia (TD). The symptoms of TD, which may become permanent, are involuntary and repetitive movements of the face, jaw, tongue/mouth, limbs and trunk.
