Prescribed to treat serious bacterial infections, Levaquin (levofloxacin) is an oral or injectable antibiotic medication that has been approved by the U.S. Food and Drug Administration (FDA) since December 1996. Some of the specific types of bacterial infections Levaquin is prescribed to treat include (but aren't limited to) ear, lung, prostate and skin infections.
Levaquin is produced by PriCara, a sub-division of Ortho-McNeil-Janssen, and is classified as a fluoroquinolone or quinolone antibiotic. As of 2007, Levaquin is the leading quinolone antibiotic medication prescribed worldwide. In 2007, it earned PriCara roughly $1.6 billion in revenue.

Given the range of such severe Levaquin side effects, the FDA has issued eight warnings regarding the possibility of developing life-threatening injuries when taking Levaquin. The last of these FDA warnings, issued in October 2008, required PriCara to include “black box” warnings on all Levaquin packaging. Black box warnings are the most serious level of FDA advisories, as they inform the public that a medication has the potential to cause deadly side effects. However, as late as July 2009, the public was still filing reports that Levaquin medication guides failed to include black box warnings.
